Flu Testing

Vitamin D Testing

Within two weeks you will be contacted by the pharmacy to set up a follow up appointment where we will walk you through your results. Fitchburg Family Pharmacy’s services platform is hosted by WorkFlow Services.
To provide these services each person being tested must build a profile with the our services database. (One Time)
To complete this profile you will need a copy of your State Issued ID and your medical insurance card and a valid credit/debit card. Your card will be charged for services that are not covered by insurance. You will need to take a picture of both of these items front and back, with your smartphone during the registration process. If using a computer to enter the data, you can type it in.
Once you have completed the online forms, you will be sent a QR code. Please bring that QR code to the so we can provide you with the service.
