Above is the clinical services set offered to all at Fitchburg Family Pharmacy in their Healthy Choice Membership. 

Services Membership: $30 a month (Paid Quarterly) and provides a member with the following value.

Drug Pricing Only Membership: $10 a month (Paid Quarterly) provides a member with the pricing described below, but no services benefits.

• - Cost Plus Pricing on all prescription drugs (NADAC=Cost)  All prescriptions will be sold to members at $10 over our cost.  Prescriptions will be filled at 90 day intervals if possible.  Controlled substances are not included in this model.

• - Controlled Substance Cost Plus Pricing (NADAC=Cost)  All controlled substances will be sold to members at $25 over our cost.  Prescriptions will be subject to review of the PDMP and refills will be restricted by the prescriber and the professional judgment of our pharmacists.

• **  NADAC is the National Average Acquisition Cost and can be looked up online for transparency.
• **  We reserve the right to adjust cost when we are unable to purchase a drug at NADAC.

All services posted on the services menu above are included in the Services Membership value added program.
