At Fitchburg Family Pharmacy we offer assistance for patients who take injected medications. These services require a prescription from your provider. Our Injection Assistance Services are available for any injectable prescription including Abilify, Aristada, Fluphenazine, Haloperidol, Invega, Insulin, Vitamin B-12, and Vivitrol. All patients must submit patient data into the WorkFlow Services platform.

To complete this profile, you will need a copy of your state-issued ID and your medical insurance card, and a valid credit/debit card. Your card will be charged for services that are not covered by insurance. You will need to take a picture of both of these items, front and back, with your smartphone during the registration process. If using a computer to enter the data, you can type it in.

Once you have completed the online forms, you will be sent a QR code. Please bring that QR code, so we can provide you with the needed services.
