All recipients must schedule an appointment and complete our online
consent form. You will be automatically redirected after you schedule your

At Fitchburg Family Pharmacy we are administering vaccines to all eligible people. If you are above the age of 6 months please register below to get your updated COVID vaccine! You can learn more at:

If you are ready to receive your COVID-19 vaccine (6 months+) please click below to schedule an appointment for your vaccine. Then you will be prompted to register for your vaccine. Please bring your white vaccine card, ID, and Insurance card to the appointment. We have the following COVID vaccines available:

• Spikevax 12 years and up

• Spikevax 6 mo to 11 years

• Novavax 12 years and up (This is only offered on specific days, watch our Facebook page for announcements)
ATTENTION: Any child under 3 years must have a prescription for each dose of a vaccine. The parent or guardian of the child must contact their medical provider who must send a prescription into the pharmacy prior to each dose.
Fitchburg Family Pharmacy’s services platform is hosted by WorkFlow Services.
To provide these services each person being tested must build a profile with
our services database. (One Time) Once you have completed the online
forms, you will be sent a QR code. Please bring that QR code to the so we can
provide you with the service.
